You do not need a doctor’s prescription to receive one of our treatments. However, if you are already seeing a doctor for an injury, we recommend that you download our prescription form and bring it to your doctor to fill in. This is the best way to ensure that you will get our high-quality service.
Our other patient forms are also available in a downloadable format so that you can fill them out prior to your consultation. Bring in a completed patient information form, health questionnaire, and consent for treatment in order to make your first meeting with our physical therapy assistant run more smoothly.
